Medical Facilities
Most providers we have worked with, work in different locations throughout their typical work week. Some have multiple medical office locations, or they work rounds at the hospital. In this case it is required that these additional locations be added in the Facilities section of the database. Without this information CMS-1500 or 837P / 837I electronic claims will go out with the incorrect Place of Service causing billing complications.

In order to setup a Medical Facility a practice would need to have the Basic Facility information including the License number, Medical Facility ID and Facility NPI number. If this is a Laboratory Facility you would need the Medical Facility CLIA number also.
